NOTE* Individuals purchasing this service will need to order a new copy of their 'LONG FORM' Birth Certificate from the state they were born. Keep in mind, this step is not necessary if you already have one in excellent condition. Just be sure to order an additional copy for your personal possession due to this being a lengthy process.

Authenticating your Birth Certificate can be a tedious and time consuming process. Many states, counties, cities and or municipalities routinely change their processes to make it harder for you to complete this process, but this is your right! After purchase, you will receive an email with simple instructions and an attached form to provide us with your general information needed in order to get the process started. You will also be sent an agreement with terms to read over and accept/sign prior to beginning your process.

We will never ask for your Social Security number or any other intrusive information because it is not needed for this process.

You will then need to mail in your birth certificate to Phoenix Planning Hub and let us take care of the rest. THIS PART IS THE SAME AS MAILING IN YOUR BIRTH CERTIFICATE ATTACHED TO YOUR PASSPORT APPLICATION......IT GETS MAILED BACK TO YOU WHEN COMPLETE.

Please keep in mind that this process can generally take 2-6 months and may be delayed due to COVID-19 restrictions.

You will receive notifications by email as updates are available.

At the end of this process, your birth certificate will have been mailed back to you with a U.S. Department of State Certified Seal attached that represents your Birth Certificate is entitled to "Full Faith and Credit".
